See if there is a firmware upgrade available. Sometimes the latest firmware will fix issues that they don't even list in the release notes.

Another thought is to set it back to factory defaults.

You can also try removing both the rechargeable battery and the system battery for 10 minutes.

Yes, I can see when I review them visually on the camera lcd screen that the 2 exposures were the same, and when I load them in Photomatix I get the message that the first two have the same exif data. I haven't used AV mode much for this, mostly A-DEP and have shot hundreds of bracketed photos in A-DEP and it never happened. What does happen sometimes it only shoots two photos instead of 3. Some modes you can't do bracketing.

maybe it was at its limit and that's all it could do.

i'd try to shoot a few more sets in different conditions. might be a fluke. otherwise write to canon and see what their view is.

What was the length of the exposure in the 2 same shoots? I know that my camera will not expose for longer than 30 seconds in aparature priority.
I have a nikon BTW.

I have seen this when close to the limits of shutter speed settings using aperture priority. One bracket just lies outside of the setting range.

I've deleted the images but I think it may have something to do with the settings in AV mode.

l found similar with my Nikon D700 - it boiled-down to timeout settings
which may not have been obvious from the beginning (and also LENR).

Auto Meter OFF Delay - if you have similar, set to No Limit.
Auto Bracketing set to AE only.
No Auto ISO - Native Value.
Auto Bracketing Mode - Flash/Aperture.
Wide Area DF Points - in my D700 this is set to 51 Points 3D tracking.

If these AF points are too specific, small changes might not be registered
as you bracket. And of course, no AF Timeouts - or set to them No Limit.
